Patient Registration

The practice only accepts new patients whose permanent residence is an address within our catchment area

Prospective patients need to complete a New Patient Questionnaire and a GMS 1 form.

These are provided at the surgery and can be completed either in the surgery or at home

If you are unable to supply a current NHS number, we will ask you supply a suitable form of Photo ID e.g. Passport, Driving Licence. 

We also accept Online Pre - Registrations if you hold an existing NHS Number.  (Please see below). Patients will be asked to sign the GMS1 at the time of there first appointment.

An appointment with a Nurse may be necessary if you are on repeat medication

If you move outside our catchment area you may need to register with the local surgery

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

When you register you will also be asked to fill out a Medical Questionnaire. This is because it can take a considerable time for us to receive your medical records. You can obtain a questionnaire in the surgery or download the questionnaire below
